By using the Healthy Balance Direct Care website, patient portal, or in‑clinic services, you agree to these Terms & Conditions (“Terms”). If you do not accept them, please do not use our site and instead contact our office.

B. Website Content Is Not Medical Advice

Educational articles, blog posts, or FAQs on our site are general information, not individualized medical advice. Always consult a qualified clinician for diagnosis or treatment.

C. Eligibility and Registration

You must be at least 18 years old—or a legal guardian acting for a minor—to create a patient‑portal account. Provide accurate information, keep it current, and safeguard your password; you are responsible for activity under your account.

D. Membership Agreement

Enrolling in an Individual, Family, or Business Membership requires signing a separate written contract that details covered services, fees, exclusions, termination rights, and refund rules. If any conflict arises, that signed agreement overrides these Terms.

E. Financial Responsibility

Prices shown online or in print may change until you sign an agreement or receive written confirmation.Payment Methods include major credit cards, ACH, or employer invoicing, as specified in your plan.Late Payments may result in reasonable service charges or suspension of non‑urgent care until the balance is paid.

F. Insurance Disclaimer

We generally do not bill third‑party insurance, Medicaid, or Medicare for membership fees. If you keep separate insurance for catastrophic events, you are responsible for submitting any claims or seeking reimbursement.

G. User Conduct

You agree not to post unlawful or offensive content, disrupt the site or networks, attempt unauthorized access, or use automated scraping tools without written permission. Violations may result in termination of portal access and, if warranted, notification to authorities.

H. Intellectual Property

All site content, including text, graphics, and software, is owned by Healthy Balance Direct Care or its licensors. You may view or print pages for personal use only; commercial use or redistribution requires prior written consent.

J. Disclaimers and Limitation of Liability
